The Challenge: 

A rapidly growing Functional Medicine practice in New England with 7 providers  was using HubSpot for marketing, lead management, and scheduling, and Cerbo as their electronic medical record (EMR) and patient management system. As the organization scaled patient acquisition and services, accurate data flow between systems became critical to operational efficiency and patient experience.

The client faced challenges maintaining alignment between prospects in HubSpot and patients in Cerbo, resulting in manual updates, inconsistent statuses, and limited segmentation capabilities.

Azure Logic Apps was selected to act as a middleware orchestration layer, enable event-driven automation, and provide logging, monitoring, and extensibility.

Key Work Patterns & Implementation

1. Automated Status Synchronization

HubSpot meeting outcomes were configured as triggers. When specific outcomes were recorded (e.g., intake completed, appointment confirmed), Azure Logic Apps automatically:

  • Updated the individual’s status in Cerbo
  • Converted the record from prospect → patient

*This eliminated manual intervention and ensured system consistency

 

2. Service Type & Therapy Tag Synchronization

Key service type and therapy-related tags were identified in HubSpot. Azure Logic Apps synchronized these values to the corresponding Cerbo fields. This enabled:

  • Accurate patient classification
  • Improved downstream reporting
  • More precise segmentation for clinical and operational analysis

 

3. Scalable, Maintainable Design

Logic Apps were designed with:

  • Clear error handling
  • Retry logic
  • Extensible workflows for future enhancements
